Support Services

Chore - Assistance with one or more of the following: heavy housework, yard work or sidewalk maintenance.

Homemaker - Preparing meals, shopping for personal items, managing money, using the telephone or doing light housework

Information & Assistance - A service for older individuals or their caregivers that:

a. Provides current information on opportunities and services available including information on assistive technology.
b. Assesses the problems and capacities of the individuals;
c. Links the individuals to the opportunities and services that are available; and
d. Establishes adequate follow-up procedures to ensure access to all services.

Legal Assistance - Provision of legal advice, counseling and representation by an attorney or other person acting under the supervision of an attorney.

Outreach - Interventions initiated for the purpose of identifying potential clients and encouraging use of existing services and benefits.

Personal Care - Providing personal assistance, stand-by assistance, supervision or cues from person with the inability to perform; eating, dressing, bathing, toileting in and out of bed/chair or walking.

Transportation - Provision of a means of going from one location to another.

Material Aid - Issuing assistive devices and other goods, e.g. walkers, wheelchairs, fans, commodities, personal hygiene items.

Medication Management - Screening and education to prevent incorrect medication and adverse drug reactions.

Sitter Service - A service provided in a home setting to ensure the health and safety of the individual. It includes observing, conversing, providing food for the individual, etc.

Telephoning - Contacting individuals by phone on a routine basis to determine physical status, to provide comfort and help.

Wellness - Activities designed to provide services which will support and/or improve the older persons' mental and/or physical well-being, e.g. exercise/physical fitness and health screening.
